I find it interesting that the general "excuse" offered is that nobody could have conceived of the notion of suicide pilots crashing airliners into public buildings.

Tom Clancy wrote a book, released in paperback in 1997, in hardback before that I think, called "Executive Orders". Here is a snippet of Amazon's review:
