In the UK you get "diagnosed" with H1N1 by calling a call centre, they then authorize you to receive a Tamiflu dose (if you want it).

However, they do then follow up a random sample of the people "diagnosed", sending them a test kit to return so that they can track what proportion of people phoning actually have H1N1. IIRC at the start of the outbreak it was just over 1 in 10.
