Do they usually come with a crossover cable or straight cable?

They vary. I've had a total of four and I think they were split two and two.

If you get a SOHO router/firewall to put between your LAN and the DSL modem (something you should do anyway), they often come with a crossover select switch right on the port.

For that matter, many switches can now autodetect whether a connection is straight or crossed.

Just for fun my D-Link DSL-300G+ came with both (straight through for direct connection to the PC or crossover for connection to a hub / router).
