Your boot log is a bit scrambled. You probably are using cut and paste from hyperterm. You are better off capturing the results to a file and then cutting / pasting. It is much clearer.

I suspect that the hard drive cable may have loosened from the header, a hard drive jumper fell off (if using the regular / too large desktop drive jumpers), that the cable might be bad or that there is corruption on the hard drives.
