I can hold the phone under one ear with my shoulder, which invariably means that a couple of times a month I wake up the next day with excruciating pain from pulled muscles in my neck, which then takes several days to go.

That's probably the least of your worries, I read an article about increasing the risk of a stroke when you hold a phone like that. Something about stretching and squashing blood vessels or something like that.

As mad as it sounds I've regularly stick the phone to my head with a large elastic band during our pointless weekly phone conferences at work. Leaving me to get on with actually doing the job instead of talking about it. The speakerphone on my desk is useless with the 6 or so PC's and associated fan-ridden equipment on my desk, plus the unbelievably loud air handling equipment in the office. So when I'm just listening I turn the microphone off. My boss refuses to buy me a headset.

EDIT: Maybe should point out I do that on my own, so nobody sees it.
